Once completed and hand-signed, below forms can be delivered to the Dean’s Office in one of the following ways:

  • In person – or by someone you authorized to act on your behalf – during Dean’s Office opening hours
  • Via traditional post service or a courier company
  • Via e-mail to – in this case all relevant original paper documents need to be scanned and attached as pdf or jpg files. Please make sure that your email subject clearly indicates type of application and contains your student number.

Please note that applications which do not meet formal requirements will not be processed. We do not accept incomplete applications, applications submitted via USOSweb system and forms missing hand signature or date.

Due to the current epidemiological situation and the need to reduce the risk of COVID-19 transmission, it is strongly recommended to send your enquiries via email to Individuals with particularly urgent matters are welcome to the Dean’s Office on Tuesdays from 10am to 12pm upon prior arrangement.
